Information needed for review to admit someone from home.
  • Physician office visit notes (if available)
  • Recent history and physical
  • Financial information, including: Medicare, Medicaid, private insurances, managed care, etc.
  • If recent hospitalization, information from that stay to include history and physical, nurse's notes, therapy notes and discharge summary
  • Medical Power of Attorney; Durable Power of Attorney; Advance Directives

There are multiple ways to pay for senior housing or long term care.

We gladly accept Medicaid, Medicare and Private Funding.

As defined in Title XIX of the Social Security Act, Medicaid is a joint Federal-State program which pays for medical services to eligible needy and vulnerable families and individuals. Medicaid currently pays for 60% of nursing facility care.

As defined in Title XVIII of the Social Security Act, Medicare ("Health Insurance for the Aged and Disabled") is a Federal health insurance program for aged (65+) and certain disabled individuals regardless of income.
